Now, exhortations to not spit may have been put up to remind people of common manners - but, like you said, also to help stop the spread of disease.
Spanish Flu came through the country around 1918ish - you can damn well bet that public health would have been a top priority.
So..... find out when they put the signs up (signs? They're stencils!) and you'll have a better guess at what the original intent was.
